Mobile app now available to Burlington residents seeking local health & social services

IDENTOS announces the completion of yet another health access project between IDENTOS and the Burlington Ontario Health Team (OHT). The project includes the launch of a Burlington Health Navigator app - a digital front door that provides 24/7 access to and discovery of health services to the residents of the Burlington community.

Now available on Apple and Google Play stores, the Health Navigator app was designed by and for patients, families and caregivers in collaboration with the Burlington OHT and technology partner IDENTOS. With a swipe or a tap, the app connects residents to a wide variety of health and social services, including help finding: 

-A family doctor or hospital care
-Mental health or addiction services
-Home care
-Chronic disease support
-Social connection
-Palliative care
-Indigenous and LGBTQ2S+ resources

“This has been a journey of many firsts, and the Burlington Health Navigator App and website is a significant milestone. It represents the first phase of establishing a ‘Digital Front Door’ to navigate local healthcare services and programs. This work also exemplifies the Burlington OHT’s vision of co-creating, with our community, better ways to access care by removing informational barriers,” said Burlington OHT co-chairs Eric Vandewall (President and CEO of Joseph Brant Hospital) and Dr. Harpal Singh (physician lead).
